Life stage transition support helps NDIS participants plan for and manage major life changes — such as finishing school, moving into new living arrangements, or starting work — so the change feels manageable rather than overwhelming.
Common transitions this support covers
The most common examples are the move from school into further study or work, a change in living arrangements (such as moving toward more independent living), and planning ahead for future support needs as circumstances change.
Support at this stage is less about a single task and more about ongoing planning — breaking a big change into smaller steps, and coordinating with family members or support coordinators along the way.
Why timing matters
Transitions tend to go more smoothly when planning starts well before the change happens, rather than in the weeks immediately before — for example, starting to plan a school-to-work transition a year or more in advance.
